I have an Audi A4, 2.4 wagon, 87000k, when should the cam belt be replaced?
The engine appears very responsive and quiet.

The replacement period for the cam belt is every 120,000km's or 5 years whichever comes first.

To do the job to Audis recommended standards, you may be faced with a few extra's such as replacement of the tensioner pulley and guide. A water pump replacement may also be recommended as it runs off the cam belt.

Don't let the engine's performance be a guide to the cam belts durability. If it breaks there is no warning.

Its going to be an expensive repair but worth it in the long run.

A broken belt will cause severe engine damage.
